Caring For Life offers financial assistance to Warren Co. families who have a child between the ages of birth and 18 (or high school) who has a serious illness that requires medical care outside of Warren County. Items eligible for reimbursement include: hospital and doctor co-pays and bills, prescriptions, medical equipment and supplies, and travel expenses to out of Warren County hospitals and doctor offices (meals, parking, lodging, and mileage). All receipts submitted for reimbursement must include verification of dates of services. This can be in the form of a parking or cafeteria receipt, school excuse, insurance forms showing the dates of services, or the “Verification of Services” form below.
